02/23/2021 Review Closure Report Rejected for following reasons : -Release Images and release size show signs/evidence of a liquid release and the C-141 and corresponding report indicate that no liquids were released. – Release liquids and volumes need to be reviewed and adjusted to be correct on from C-141, and all instances of no liquids released in the report need to be changed to include a volume of released liquids. -Field Data, and Scaled SiteMap/Sampling Map do not represent the excavation size that OCD witnessed on the 10/1/2020 sampling event. The Site map and Photos appear to be for an event prior to the start of remediation when the pipeline was stripped back and repaired – Please correct the scaled site/sampling map to include lengths/widths/ and depths of the completed excavation. -Included photos do not show the remediated site prior to backfill. - Please include photos of the excavation prior to backfill from the 10/1/2020 or the 10/5/2020 sampling events. -Please correct, review and resubmit a complete C-141 final report no later than March 26, 2021 -Missing Page 1 of Form C-141

10/16/2020 The (02/23/2021, C-141) application [10667] was assigned to this incident.
10/01/2020 Onsite for soil sampling. The excavation was approximately 25x20x13 feet deep. The Operator collected 6 5pt composite samples, 1 from each side wall and 2 from the base. There was light Hydrocarbon odor from the bottom soils and no obvious signs of stains. One thing of note is that the Pipeline to the east is new coated pipe and the pipe to the west is older degraded pipe. I suspected that the operator had the pipeline stripped back further then what we were seeing today. I called Kijun Hong is ask when how much line was replaced and when. Kijun reported that 42 feet of pipeline was replaced around August 18th.
09/30/2020 Operator notified of sampling events for 9/1/2020
09/29/2020 The (09/29/2020, C-141) application [10390] was accepted by OCD. The operator was emailed with details of this event.

08/13/2020 C-141 received on 9/28/2020 for release on 8/13/2020. The cause of the release was reported as “Natural gas pipeline leak, no liquids. Site is within an unnamed wash that is a tributary to Armenta Canyon wash. A line occurred on the well tie line for the Hargrave 1. The line blew down through leak. Leak has been repaired.”
